Student Enrollment Process/Forms

Welcome to Holton Public Schools!

We look forward to working with you and your child and welcome you to the Holton Public Schools family! Please see below for important information regarding the enrollment process and required documentation.  Please be aware that your child may not attend class until the proper documentation is on file with the school district.

Enrollment Process:

Step 1 - Online Registration

NOTE:  The online enrollment form requires an overnight to process to our database.  Please complete the form one business day before you intend to visit your building office with your documents. Please contact your building office to complete the process; we are not aware that you have registered until you let the office know you have started the process, and you must complete steps 2 & 3.  Thank you!

Step 2 - Gather Required Documents

We will make copies of your documents and/or print out any forms you need at the building.  An enrollment packet (other documents) will be provided to you at the building - you do not need to print it.

Elementary (Kindergarten - 5th grade):

  • Original Birth Certificate
  • Current Immunization Record 
  • Proof of Residency 
  • Proof of a hearing & vision screening (Kindergarten only)
  • Other documents requiring your signature will be provided to you at the building

Middle School (6th - 8th grade):

  • Original Birth Certificate
  • Current Immunization Record 
  • Proof of Residency 
  • Other documents requiring your signature will be provided to you at the building

High School (9th - 12th grade):

  • Original Birth Certificate
  • Current Immunization Record 
  • Proof of Residency
  • Recent Transcript (or recent 8th grade report card) 
  • Other documents requiring your signature will be provided to you at the building

Holton Virtual Academy (9th - 12th grade):

  • Original Birth Certificate
  • Current Immunization Record 
  • Proof of Residency
  • Recent Transcript (or recent 8th grade report card) 
  • Other documents requiring your signature will be provided to you at the building

Muskegon County Virtual Academy (Kindergarten - 12th grade):

Step 3 - Take your required documents to the building

Elementary:  6425 Syers Rd, Holton, MI 49425

School Based Health Services

According to data from the National School-Based Health Alliance, when students are healthy, they miss less school and concentrate better in class . 

Holton Public School District believes healthy students are better learners, so we have partnered with Hackley Community Care School Based Health Program to offer behavioral health services to students within the district.  Most Hackley Community Care School Based Health Services require parental consent, Hackley Community Care School Based Health Consent Form.
